AngelWings Health Care is here for you. We offer care following an injury, an illness, hospital/facility stay,
surgery and or child birth. Care can be provided in your own home or in assisted living facilities. By
combining expert clinicians, innovative systems, access to our continuum of care and commitment to
exceptional service, we are able to consistently deliver an affordable high quality of care.

CARE – Delivery Guidelines

Our care packages comprise three care levels. They include:
 Low Care;
 High Care; and
 High Care Dementia

Low Care

Low Care is for older people with low level complex care needs who wish to remain living in their own homes and are able to do so with the assistance of our care package. This includes
residents of retirement villages.

Low Care packages are individually planned and coordinated packages of community aged care services, targeted at frail older people living in the community who require management of services because of their complex care needs. These people would otherwise be eligible for at least low level residential care.

High Care

The High Care level is a community alternative to high level residential care by providing the equivalent of high level residential care to frail older people with high level complex care needs, who wish to remain living in their own homes, and are able to do so with the assistance of our care package. this includes residents of retirement villages.

High Care packages are individually planned and coordinated packages of community aged care services provided to care recipients. These packages differ from the Low Care packages in
that they are specifically targeted at frail older people living in the community who would otherwise be eligible for high level residential aged care.The packages are flexible in content, however the expectation is that a package would include qualified nursing input, particularly in the design and ongoing management of the package.

High Care Dementia

The High Care Dementia level is a community alternative to high level residential care. These packages are individually planned and coordinated packages of care tailored to help frail older people who have been assessed as having high complex care needs and experiencing behaviors of concern and psychological symptoms associated with dementia.

Difference between High Care and High Care Dementia

The key difference between a High Care and High Care Dementia package of care is that High
Care Dementia specifically targets the behaviors of concern and psychological symptoms
associated with dementia and provides methodologies and services that are targeted towards
managing these behaviors.
